Job summary

Paradigm is seeking a Field Nurse Case Manager to cover the South Bay Area in CA and surrounding areas. This home-based role involves travel up to 2 hours one-way to doctors’ offices, hospitals and other locations, with potential multiple appointments in a day.

You will perform medical case management for work-related injuries, including assessment, planning, coordination, implementation and evaluation, working with insurers, providers, attorneys, employers, and employees, and reporting progress.

Qualifications

• Current, unencumbered RN license in California.
• Experience in workers' comp case management preferred.
• Certifications CCM, CRRN, CDMS, CRC strongly preferred; must obtain within two years.
• Strong organizational and communication skills; able to work autonomously.
• Valid driver’s license and good driving history.

Responsibilities

• Manage medical case coordination for workers' comp injuries.
• Assess, plan, coordinate, implement, and evaluate injured workers.
• Liaise with insurance carriers, medical providers, attorneys, employers, and employees.
• Monitor progress and report results to employer and carriers.
• Travel to doctors' offices, hospitals, and other locations.
