Job summary

CVS Health is seeking a Case Manager Registered Nurse - Field to support members in Essex County and surrounding areas. This role requires home visits, care planning, and collaboration with a multidisciplinary team to ensure high-quality, cost-effective services.

The ideal candidate holds an active NJ RN license with 3+ years of clinical experience and is able to travel extensively while maintaining exceptional communication and documentation in the EHR system.

Qualifications

• Active, unrestricted RN license in NJ.
• Minimum 3+ years clinical practice in hospital or similar setting.
• Willing to travel up to 75% in Essex County and surrounding areas.

Responsibilities

• Conduct face-to-face home visits for MLTSS/D‑SNP/FIDE members.
• Develop and adjust care plans with care team and members.
• Coordinate with PCP and care team for appropriate services.
• Document care activities accurately in the EHR.
• Mentor new hires after proficiency is reached.
