Growing Home Care Agency is seeking a full-time Field RN Case Manager to join our dynamic team. This position will play a vital role in providing high-quality and compassionate healthcare services to our clients and their families.

The Field RN Case Manager will conduct initial assessments, perform regular home visits, and collaborate with the interdisciplinary team to ensure proper patient care. You will provide education and support, maintain records, and participate in care strategy

Qualifications

• Current NYS RN license.
• One year of nursing experience preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassion and dedication to patient-centered care.
• Ability to multi task.
• Reliable transportation and valid driver's license.

Responsibilities

• Conduct initial assessments of new clients to determine healthcare needs and develop individualized care plans.
• Perform regular home visits to assess client progress and adjust care plans as needed.
• Collaborate with the interdisciplinary team to ensure proper patient care.
• Provide education and emotional support to clients and families.
• Maintain accurate patient records and documentation.
• Provide in-service trainings and guidance to Home Health Aides and Personal Care Assistants.
• Participate in team meetings and contribute to care strategies.
