Job summary

UnitedHealth Group is seeking a Registered Nurse for a field-based role serving the greater Washington, D.C. area. You will visit members in homes or facilities, coordinating care and implementing individualized plans with an interdisciplinary team.

Responsibilities include assessing needs, developing care plans, and communicating with providers and families to ensure quality, seamless care. The role emphasizes advocacy, adherence to regulations, and effective use of EMR systems.

Qualifications

• Active RN license in DC or ability to obtain within 90 days.
• BLS certification.
• 2+ years of experience using MS Word, Excel and Outlook.
• 1+ years of post-acute care experience such as long-term care.
• Experience with Electronic Medical Records.

Responsibilities

• Assess, plan and implement care management interventions individualized for each member.
• Develop and implement care plan interventions across the continuum of care as a single point of contact.
• Communicate required health information to stakeholders for coordinated care.
• Advocate for members and families to ensure needs are represented by the care team.
• Identify gaps or barriers in treatment plans based on consumer needs.
• Document the plan of care in appropriate EHR systems.
• Maintain engagement by building rapport and empathy with members.
• Collaborate with providers to align goals in the plan of care.
