• Familiarity with EMR and health IT tools.

Responsibilities

• Manage a caseload and complete in-home or telephonic assessments.
• Develop and coordinate care plans with the care team.
• Review medical complexity attestations and clinical oversight tasks.
• Ensure services are in least restrictive settings for member well-being.
• Involve the care team in care planning and coordinate with external resources.
