Qualifications

• Must reside in the state of Illinois.
• Registered Nurse in Illinois without disciplinary action.
• 2+ years of in-home case/care management.
• Experience working with the adult population.
• Knowledge of community health and social service agencies and resources.
• Exceptional communication and interpersonal skills with the ability to quickly build rapport.
• Ability to work with minimal supervision within the role and scope.
• Ability to use electronic information applications/software including EMR.
• Intermediate to Advanced computer skills with Word/Outlook/Excel.
• Excellent keyboard and web navigation skills.

Responsibilities

• Identifies and resolves barriers to effective care.
• Monitors patient care through assessments and evaluations.
• Creates plans of care.
• Communicates with internal and external stakeholders.
• Understands department and organizational strategy and linking to related areas.
• Makes decisions with minimal direction.
• Follows established guidelines and procedures.
• Visits Medicaid members in their homes, supportive living facilities, and other care settings (75-90% local travel).
