Job summary

LivWell Behavioral in Chandler, AZ is seeking a Community Resource Specialist to coordinate home deliveries of resource packets, food bags, and essentials. This full-time role blends field delivery routes with behind-the-scenes coordination with care teams to ensure families receive what they need.

You will drive 3–4 routes weekly, utilize Microsoft 365 tools, and document activities accurately. The position offers a Monday–Friday schedule with early Friday releases and a company vehicle for

Qualifications

• High school diploma or equivalent is required.
• Valid driver’s license and ability to travel within the service area.
• Proficiency with Microsoft 365 and willingness to learn new platforms.
• Strong written and verbal communication skills.
• Excellent organization, time management, and problem-solving abilities.
• Ability to work independently while collaborating with a team.
• Maintain professional boundaries and sound judgment.
• At least one of: healthcare setting experience, health education cert, or related college coursework.

Responsibilities

• Coordinate weekly distribution of resource packets, food bags, and resource bags.
• Prepare delivery materials and assemble bags.
• Deliver resources to clients' homes using company vehicle, 3–4 routes per week.
• Ensure distributions are accurate and complete on time.
• Document notes thoroughly in a high-volume environment.
• Use Microsoft 365, Power Automate, and Canva to support communication and program activities.
• Maintain regular communication with Process of Care Department and Case Managers.
• Participate in trainings, supervision, and monthly one‑on‑ones.
