Job summary

LivWell Behavioral in Chandler, AZ is seeking a Community Resource Specialist for a full-time role that blends field delivery of resource packets and behind-the-scenes coordination to help families in need. The position runs Monday–Thursday 8:00am–4:30/5:00pm with Friday early release and a company vehicle for routes.

You will document activities, work with care teams, and be hands-on in behavioral health while serving children and families in the community.

Qualifications

• High school diploma or equivalent.
• Valid driver's license and the ability to travel throughout the assigned service area.
• Proficiency in Microsoft 365 and comfort learning new technology platforms.
• Strong written and verbal communication skills.
• Solid organization, time management, and problem-solving skills.
• Ability to work independently while contributing to a collaborative team.
• Sound judgment and the ability to maintain professional boundaries.

Responsibilities

• Coordinate and support the weekly distribution of resource packets, food bags, and resource bags.
• Prepare materials for delivery by printing and organizing resource packets, creating recipe cards, purchasing supplies, and assembling bags.
• Deliver resources directly to clients' homes using a company vehicle, typically 3-4 routes per week lasting about 3-4 hours each.
• Make sure every distribution is organized, accurate, and completed on time.
• Complete accurate, detailed, and timely documentation while managing a high volume of notes.
• Use Microsoft 365 (Outlook, Word, Excel, Teams, SharePoint), Power Automate, and Canva to support communication, tracking, and program activities.
• Stay in regular communication with the Process of Care Department and Case Managers about client needs and service coordination.
• Participate in trainings, supervision sessions, staff and collaboration meetings, and monthly one-on-ones with your supervisor.
