Legacy Home Care seeks a Community Liaison to grow private pay home care referrals in the Greater Twin Cities area. You will work with hospitals, senior living communities, and care teams to identify discharged clients needing support and position Legacy as the trusted provider.

You will build lasting relationships, educate sources and families on options, and manage a repeatable referral process. A true hunter who works independently and hits metrics will thrive here.

Qualifications

• Bachelor’s degree is required.
• Healthcare business development experience preferred.
• Ability to build referral relationships and turn conversations into revenue.
• Proven prospecting and networking skills.
• Excellent communication in person, by phone, and in writing.

Responsibilities

• Build referral relationships with hospitals, social workers, discharge planners, and senior living leaders.
• Identify families who may need private pay care after hospitalization or decline.
• Educate referral sources and families about private pay care options.
• Create a repeatable referral process with consistent outreach and follow-up.
• Attend networking events to increase brand visibility.
• Track activity, leads sources, and conversions.
