Job summary

CareBuilders at Home seeks a motivated Community Liaison Representative to grow private-duty home care and senior placement services. The role targets relationships across hospitals, SNFs, rehab centers, and senior living communities to drive referrals and placements.

You will cultivate connections with discharge planners, social workers, case managers, and network within post-acute settings to expand our non-medical home care and placement offerings.

Qualifications

• Experience in healthcare, senior care, discharge planning, case management, facility admissions, or post-acute outreach.
• Ability to build trusted relationships with hospitals, SNFs, rehab centers, and senior living communities.
• Proven revenue-generation capability in home care or placement services.

Responsibilities

• Develop and maintain referral relationships with hospitals, SNFs, rehab centers, and senior care communities.
• Identify client and family needs during transition points and introduce CareBuilders at Home services.
• Generate qualified referrals that convert into home care or placement business.
