Job summary

Amcare Pro Home Health, Palliative Care & Hospice in the Dallas area seeks a compassionate Registered Nurse for a PRN field position delivering in-home visits across DeSoto and nearby communities. The role focuses on skilled nursing, assessments, and care coordination.

You will travel to patient homes, complete Start of Care and recertifications, manage medications, wound care, IV therapy, education, and documentation, while collaborating with physicians and the care team.

Qualifications

• Active Texas RN license required.
• Current CPR or BLS certification.
• At least 1 year of nursing experience; home health/hospice preferred.
• OASIS and WellSky/Kinnser experience is a plus.
• Valid Texas driver’s license and reliable transportation.
• Ability to travel across DeSoto, Duncanville, Oak Cliff and surrounding areas.

Responsibilities

• Perform skilled nursing visits in patients' homes
• Complete Start of Care, Recertification, routine and discharge visits as assigned
• Conduct comprehensive patient assessments
• Develop and update individualized plans of care
• Administer medications and treatments per physician orders
• Provide wound care, IV therapy, disease management and other skilled nursing services
• Educate patients and caregivers regarding diagnoses, medications, treatments, safety, and care plans
• Monitor patients for changes in condition and communicate concerns
• Coordinate care with physicians, therapists and the interdisciplinary team
• Complete accurate OASIS and clinical documentation
• Follow agency policies and Medicare/state/federal requirements
• Travel to patients across DeSoto, Duncanville, Oak Cliff, Lancaster and nearby areas

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dependable Registered Nurse for a PRN field position serving patients throughout South Dallas, including DeSoto, Duncanville, Oak Cliff, Lancaster, and surrounding communities.

• OASIS and WellSky/Kinnser experience is a plus


• Strong assessment, clinical decision-making, and time-management skills


• Proficiency with electronic medical record systems


• Valid Texas driver's license


• Reliable transportation and current automobile insurance


• Ability to work independently and collaborate with the clinical team
