Job summary

A home health care provider in Texas is seeking a dedicated Home Health RN Case Manager for PRN or Full-Time positions. Ideal candidates will have strong home health experience, effective communication skills, and a valid Texas RN License. Responsibilities include conducting field assessments, providing skilled nursing care, and ensuring compliance with state regulations. Candidates should possess at least 2 years of nursing experience and be prepared to work in the South Dallas area, including Duncanville. Qualifications

• Must currently be licensed in the state of Texas.
• Should have at least 2 years nursing experience.
• Prior home health care experience is a MUST.
• IV infusion experience preferred; driving and transportation required.
• Strong communication and ability to educate patients and families.

Responsibilities

• Conduct SOC, ROC, recertification, and discharge visits.
• Complete OASIS assessments accurately and timely.
• Provide skilled nursing care including wound care and medication management.
• Provide skilled nursing care (wound care, medication management, IV therapy as needed).
• Coordinate care with physicians and interdisciplinary team.
• Educate patients and caregivers on treatment plans and safety.
• Ensure compliance with Medicare and Texas home health regulations.
• Complete documentation in the agency EMR within required timeframes.

Job Description

Amcare Pro Home Health is a mid-size home health care company with offices in DFW & East Texas. The DFW office is currently interviewing for a Home Health RN Case Manager PRN or Full Time to cover the South Dallas areas (Duncanville, Cedar Hills, Desoto, Lancaster, Hutchins & surrounding areas).

The position will involve home care visits for Admits, Recerts, Sup & SN visits. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/AXXESS) is a plus.

Job Details

The RN position will be required to perform home health SN Initial Assessments, OASIS Admissions, Re-cert & ROC visits, DC visits, and ensure compliance with all State regulations. The RN must have knowledge/experience with home health forms, OASIS charting, Medicare guidelines, rules & regulations.

• Complete all documentation in the agency EMR within required timeframes

Qualifications

1. Must currently be licensed in the state of Texas

5. Should have reliable transportation & car insurance
6. Must be able to pass background check & have valid CPR
7. Experience with Oasis E and use of Wellsky/Kinnser is a plus
8. Knowledge of ICD-10 codes is a plus
