Job summary

Amcare Pro Home Health, Palliative Care & Hospice in Kaufman, TX is seeking a compassionate Licensed Vocational Nurse for a PRN field role serving Kaufman, Terrell and surrounding communities. The LVN will perform skilled nursing visits, administer medications, educate patients and caregivers, and document care while traveling within the service area.

The role requires an active Texas LVN license, CPR/BLS, strong clinical and communication skills, and the ability to work independently under RN

Qualifications

• Active and unrestricted Texas LVN license.
• Current CPR or BLS certification.
• At least one year of nursing experience preferred.
• Home health, hospice, or palliative care experience preferred.
• WellSky/Kinnser experience is a plus.
• Strong clinical, communication, and time-management skills.
• Valid driver’s license and reliable transportation.
• Ability to work independently under RN supervision and travel within the service area.

Responsibilities

• Perform skilled nursing visits in patients’ homes.
• Provide nursing care per physician orders and plan of care.
• Administer medications and treatments within LVN scope.
• Monitor patient conditions and report changes to RN or physician.
• Educate patients and caregivers on medications, treatments, safety, and care.
• Communicate with the supervising RN and care team regularly.
• Complete accurate and timely clinical documentation.
• Follow infection-control procedures and agency policies.
• Travel to patients throughout Kaufman, Terrell, and surrounding areas.

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dependable Licensed Vocational Nurse for a PRN field position serving patients in Kaufman, Terrell, and surrounding communities.

Our Licensed Vocational Nurses provide skilled, patient-centered care in the comfort of each patient's home. This role supports patients in safely managing their conditions while maintaining their independence and quality of life.

Responsibilities

• Perform skilled nursing visits in patients’ homes
• Provide nursing care according to physician orders and the established plan of care
• Administer medications and treatments within the LVN scope of practice
• Monitor patients’ conditions and promptly report significant changes to the supervising Registered Nurse or physician
• Educate patients and caregivers regarding medications, treatments, safety, and care instructions
• Communicate regularly with the supervising Registered Nurse and interdisciplinary care team
• Complete accurate and timely clinical documentation
• Follow infection-control procedures, agency policies, and applicable regulations
• Travel to patients throughout Kaufman, Terrell, and surrounding areas
