Job summary

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate RN for a PRN field position serving patients throughout Fort Worth, Arlington, and surrounding communities. You will provide skilled, patient-centered care in patients’ homes and help manage health conditions to support independence.

The role emphasizes comprehensive assessments, individualized care plans, wound care, IV therapy, and coordination with the interdisciplinary team.

Qualifications

• Active Texas RN license is required.
• CPR or BLS certification is required.
• 1 year nursing experience preferred.
• Home health, hospice, or palliative care experience preferred.
• OASIS and WellSky/Kinnser experience is a plus.
• Proficiency with electronic medical record systems.
• Valid Texas driver’s license and reliable transportation.

Responsibilities

• Perform skilled nursing visits in patients’ homes.
• Complete Start of Care, Resumption of Care, recertification, routine, evaluation, and discharge visits.
• Conduct comprehensive patient assessments.
• Develop and update individualized plans of care.
• Administer medications and treatments per physician orders.
• Provide wound care, IV therapy, and disease management.
• Educate patients and caregivers on diagnoses, medications, and safety.
• Monitor patient condition and communicate concerns promptly.
• Coordinate care with physicians, therapists, and the clinical team.
• Ensure accurate OASIS and clinical documentation.
• Adhere to agency policies and Medicare/state/federal requirements.
• Travel to patients throughout Fort Worth, Arlington, and surrounding areas.

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dependable Registered Nurse for a PRN field position serving patients throughout Fort Worth, Arlington, and surrounding communities.

Responsibilities

• Perform skilled nursing visits in patients’ homes
• Complete Start of Care, Resumption of Care, recertification, routine, evaluation, and discharge visits as assigned
• Conduct comprehensive patient assessments
• Develop and update individualized plans of care
• Administer medications and treatments according to physician orders
• Provide wound care, IV therapy, disease management, and other skilled nursing services as ordered
• Educate patients and caregivers regarding diagnoses, medications, treatments, safety, and care plans
• Monitor patients for changes in condition and promptly communicate concerns
• Coordinate care with physicians, caregivers, therapists, and the interdisciplinary team
• Complete accurate and timely OASIS and clinical documentation
• Follow agency policies and applicable Medicare, state, and federal requirements
• Travel to patients throughout Fort Worth, Arlington, and surrounding areas

Qualifications

• Active and unrestricted Texas Registered Nurse license
• Current CPR or BLS certification
• At least one year of nursing experience preferred
• Home health, hospice, or palliative care experience preferred
• OASIS and WellSky/Kinnser experience is a plus
• Strong assessment, clinical decision-making, and time-management skills
• Proficiency with electronic medical record systems
• Valid Texas driver’s license
• Reliable transportation and current automobile insurance
• Ability to work independently and collaborate with the clinical team
