Job summary

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate PRN Field Registered Nurse (RN) to deliver high-quality in-home care across Fort Worth, Arlington, and nearby areas. You will perform skilled nursing visits, develop plans of care, and coordinate with physicians and caregivers to ensure patient well-being.

The role requires an active Texas RN license and CPR/BLS certification, with preferred experience in home health or hospice.

Qualifications

• Active and unrestricted Texas RN license.
• Current CPR/BLS certification.
• At least one year of nursing experience preferred.
• Home health, hospice, or palliative care experience preferred.
• OASIS and WellSky/Kinnser experience is a plus.
• Valid driver's license, reliable transportation.
• Strong assessment, communication, and time-management skills.
• Ability to travel throughout Fort Worth, Arlington, and surrounding areas.

Responsibilities

• Perform skilled nursing visits in patients' homes.
• Complete admissions, resumptions of care, recertifications, evaluations, and discharges as assigned.
• Assess patients and develop or update individualized plans of care.
• Administer medications and treatments according to physician orders.
• Educate patients and caregivers about diagnoses, medications, and care plans.
• Monitor changes in patients' conditions and promptly report concerns.
• Coordinate care with physicians, caregivers, therapists, and the interdisciplinary team.
• Complete accurate and timely clinical documentation.
• Follow agency policies and applicable state and federal regulations.
