Job summary

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate Registered Nurse for a PRN field position serving patients throughout Fort Worth, Arlington, and surrounding communities.

As a home health RN, you will perform skilled visits, assess patients, and develop care plans in collaboration with the clinical team. PRN scheduling offers flexibility, with opportunities for growth and professional development.

Qualifications

• Active and unrestricted Texas RN license.
• Current CPR or BLS certification.
• At least one year of nursing experience preferred.
• Home health, hospice, or palliative care experience preferred.
• OASIS and WellSky/Kinnser experience a plus.
• Strong assessment, clinical decision-making, and time-management skills.
• Proficiency with electronic medical record systems.
• Valid Texas driver’s license.
• Reliable transportation and current automobile insurance.
• Ability to work independently and collaborate with the clinical team.

Responsibilities

• Perform skilled nursing visits in patients’ homes.
• Complete Start of Care, Recertification, and discharge visits.
• Conduct comprehensive patient assessments.
• Develop and update individualized plans of care.
• Administer medications and treatments per physician orders.
• Provide wound care, IV therapy, and other skilled nursing services.
• Educate patients and caregivers on diagnoses, medications, treatments, safety.
• Monitor patients for changes and communicate concerns.
• Coordinate care with physicians, caregivers, therapists, and the team.
• Complete accurate OASIS and clinical documentation.
• Follow agency policies and Medicare/state/federal requirements.
• Travel to patients across Fort Worth, Arlington, and surrounding areas.

Responsibilities

• Perform skilled nursing visits in patients’ homes


• Complete Start of Care, Resumption of Care, recertification, routine, evaluation, and discharge visits as assigned


• Conduct comprehensive patient assessments


• Develop and update individualized plans of care


• Administer medications and treatments according to physician orders


• Provide wound care, IV therapy, disease management, and other skilled nursing services as ordered


• Educate patients and caregivers regarding diagnoses, medications, treatments, safety, and care plans


• Monitor patients for changes in condition and promptly communicate concerns


• Coordinate care with physicians, caregivers, therapists, and the interdisciplinary team


• Complete accurate and timely OASIS and clinical documentation


• Follow agency policies and applicable Medicare, state, and federal requirements


• Travel to patients throughout Fort Worth, Arlington, and surrounding areas
