Job summary

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate Registered Nurse for a PRN field position serving Kaufman, Terrell, and surrounding communities. You will provide skilled nursing visits in patients’ homes, perform assessments, and collaborate with a multidisciplinary team to support independence and quality of life.

The role requires Texas RN licensure, CPR/BLS, and strong clinical skills.

Qualifications

• Active and unrestricted Texas Registered Nurse license.
• Current CPR or BLS certification.
• At least one year of nursing experience preferred.
• Home health, hospice, or palliative care experience preferred.
• OASIS and WellSky/Kinnser experience is a plus.
• Strong assessment, clinical decision-making, and time-management skills.
• Proficiency with electronic medical record systems.
• Valid Texas driver’s license.
• Reliable transportation and current automobile insurance.
• Ability to work independently and collaborate with the clinical team.

Responsibilities

• Perform skilled nursing visits in patients’ homes.
• Complete Start of Care, Resumption of Care, recertification, routine, evaluation, and discharge visits as assigned.
• Conduct comprehensive patient assessments.
• Develop and update individualized plans of care.
• Administer medications and treatments according to physician orders.
• Provide wound care, IV therapy, disease management, and other skilled nursing services as ordered.
• Educate patients and caregivers regarding diagnoses, medications, treatments, safety, and care plans.
• Monitor patients for changes in condition and promptly communicate concerns.
• Coordinate care with physicians, caregivers, therapists, and the interdisciplinary team.
• Complete accurate and timely OASIS and clinical documentation.
• Follow agency policies and applicable Medicare, state, and federal requirements.
• Travel to patients throughout Kaufman, Terrell, and surrounding areas.
