Job summary

Monogram Health is seeking a field-based role focused on visiting patients in their homes to introduce the Monogram Health program, build trust, and enroll eligible individuals. You will travel within the territory to meet daily goals, while maintaining accurate CRM records and delivering high-quality, in-person outreach.

No healthcare experience is required; comprehensive training and support are provided.

Qualifications

• High school diploma or GED required.
• Valid driver’s license and reliable transportation; willingness to travel daily in the territory.
• Comfort with in-person, door-to-door outreach; this is a field role.
• Strong communication, relationship-building, organization, and follow-up skills.
• Self-motivation, resilience, adaptability; able to work independently toward goals.
• Nice to have: Bachelor's degree or experience in field outreach, community engagement, enrollment, business development, account management, or customer service.

Responsibilities

• Visit eligible patients in their homes and introduce them to the Monogram Health program.
• Build trust through respectful, face-to-face conversations; help enroll patients.
• Meet daily visit targets and enrollment goals.
• Route efficiently across assigned patients and conduct follow-up.
• Keep accurate CRM records and track follow-up activity.
