Monogram Health is seeking a field-based outreach professional to visit eligible patients in their homes, build trust, and guide them through enrollment into the Monogram Health program. The role emphasizes independent work, daily patient visits, and achieving enrollment targets across a defined territory.

No healthcare experience is required, as comprehensive training and support are provided. Expect travel within the assigned territory and a performance-driven environment with incentive

Formación

• High school diploma or GED required.
• Valid driver’s license and willingness to travel throughout the assigned territory daily.
• Comfort with high-volume, in-person outreach in a field role.
• Strong communication, relationship-building, organization, and follow-up skills.
• Self-motivation, resilience, adaptability, and comfort working independently toward measurable goals.
• No healthcare experience required; transferable experience encouraged.

Responsabilidades

• Visit eligible patients in their homes and introduce them to the Monogram Health program.
• Build trust through face-to-face conversations and help enroll patients.
• Meet daily expectations for patient visits and enrollments.
• Route efficiently across assigned patients and conduct follow-ups to support enrollment goals.
• Keep accurate CRM records and track follow-up activity toward monthly goals.
