Monogram Health is looking for a field-based Outreach Specialist to visit patients at home, introduce them to our program, and enroll eligible individuals. You will build trust through in-person conversations, document interactions in the CRM, and stay on track with daily visit and enrollment targets.

Responsibilities

• Visit eligible patients in their homes and introduce them to the Monogram Health program.
• Build trust through face-to-face conversations and help patients enroll.
• Work toward daily visit and enrollment targets and follow up on engagements.
• Route efficiently across assigned patients and document activity in CRM.
• Keep accurate records and stay accountable to monthly performance goals.
