Job summary

Monogram Health is seeking a Field Outreach Representative to visit patients in their homes, introduce them to the Monogram Health program, and guide enrollment. You will travel within the assigned territory, build trust, and work toward enrollment and performance goals.

The role emphasizes independence, low barriers to entry (healthcare experience not required), and training provided. Base salary is competitive with incentives and comprehensive benefits.

Qualifications

• Requires high school diploma or GED.
• Comfort with high-volume, in-person outreach, field role.

Responsibilities

• Visit eligible patients in their homes and introduce them to the Monogram Health program.
• Build trust through face-to-face conversations and help eligible patients enroll.
• Meet daily expectations for patient visits and enrollments.
• Maintain CRM records and track follow-up activity.
