Monogram Health is seeking a field-based enrollment specialist who will visit patients in their homes to introduce the Monogram Health program, build trust, and guide enrollment. You will travel daily across the assigned territory, conduct meaningful conversations, and maintain accurate CRM records to support performance goals.

Responsibilities

• Visit eligible patients in their homes and introduce them to the Monogram Health program.
• Build trust through face-to-face conversations, answer questions, and help enroll patients.
• Meet daily expectations for patient visits, meaningful conversations, and enrollments.
• Route efficiently across assigned patients and conduct follow-ups to support engagement and enrollment goals.
• Keep accurate CRM records and track follow-up activity toward monthly performance goals.
