Job summary

Monogram Health is seeking a field-based outreach specialist to meet eligible patients at home, build trust, and guide enrollment in our in-home care program. You’ll work independently in the community, travel daily, and focus on meaningful conversations that improve health outcomes.

No healthcare experience required; training provided. A high school diploma or GED is required, with strong communication, organization, and CRM skills.

Qualifications

• High school diploma or GED required.
• Valid driver's license and reliable transportation; willing to travel.
• Comfort with in-person, field outreach and door-to-door visits.
• Strong communication, relationship-building, organization, and follow-up skills.
• Self-motivated, resilient, adaptable, and able to work independently.
• Nice to have: Bachelor's degree or field outreach experience.
• CRM experience preferred.

Responsibilities

• Visit patients in their homes and introduce the program.
• Build trust through face-to-face conversations and enroll eligible patients.
• Meet daily visit and enrollment targets.
• Route efficiently between assignments and conduct follow-ups.
• Maintain accurate CRM records and track follow-ups.
