Job summary

Alameda Health System is seeking a Care Management Community Health Worker I, available to current employees only. The role is based in San Leandro, CA, with duties spanning outreach, care coordination, and health coaching within an interdisciplinary team.

Per diem day shifts and the mobile health clinic are noted by the department. The position requires a high school diploma or equivalent and experience in outreach or care coordination.

Qualifications

• Minimum High School Diploma or equivalent.
• One year public sector or health care outreach/coordination experience preferred.

Responsibilities

• Coordinate patient care activities with the healthcare team, patient, and community partners.
• Perform screenings and assessments to identify health and social barriers to care.
• Develop and maintain care plans based on assessments and documentation in the EHR.
• Provide education and connect patients to community resources to improve health outcomes.
