Job summary

Pacific Health Group is seeking a Community Health Worker (CHW) for field-based, CalAIM program support with a hybrid field/remote schedule in California. The role focuses on outreach, education, and care coordination for vulnerable populations, ensuring linkage to services and documentation in real-time.

You will travel across the county to engage members, deliver health education, assist with enrollment and navigation, and collaborate with RNs and providers in a team-based environment.

Qualifications

• High school diploma or equivalent; associate or bachelor’s degree preferred.
• Minimum of 2 years in community health, social services, or related field.
• Strong understanding of community resources and social determinants of health.
• Excellent interpersonal, communication, and engagement skills.
• Proficiency in electronic health records and basic computer systems; bilingual skills preferred.

Responsibilities

• Spend time in the field conducting outreach and engagement.
• Travel within the county to meet members and partners.
• Support health education, care navigation, and resource coordination.
• Facilitate group sessions and community-based activities.
• Document services and interactions in real-time.
• Collaborate with RNs and licensed providers in care planning.
