Job summary

Impact Life is seeking an engaging Outreach Specialist to support our Sussex County community outreach initiative. This frontline role connects individuals with treatment, recovery, healthcare, and supportive services in the community.

Responsibilities include harm reduction education, SBIRT screenings, care navigation, warm handoffs, and collaboration with local providers to improve access to MOUD and recovery supports.

Qualifications

• Knowledge of harm reduction, overdose prevention, SBIRT and related behavioral health concepts.
• Ability to engage individuals respectfully in community settings.
• Strong organizational and documentation skills, with confidentiality.

Responsibilities

• Conduct community-based outreach and engagement throughout Sussex County.
• Provide harm reduction education and distribute approved supplies.
• Perform SBIRT screenings and assist with care navigation to treatment and recovery services.
• Maintain accurate outreach records and support grant outcomes.
