Job summary

Impact Life, Inc. is seeking an Outreach Specialist to engage Sussex County communities and connect individuals with treatment, recovery, healthcare, and supportive services.

The role focuses on harm reduction education, SBIRT screening, and warm handoffs to treatment and recovery services, with travel across Sussex County and collaboration with clinical and community partners. A valid driver’s license and ability to work independently in the field are required.

Qualifications

• High school diploma or equivalent required.
• Harm reduction, overdose prevention, SBIRT, and behavioral health knowledge or willingness to learn.
• Strong interpersonal and communication skills for community settings.
• Ability to maintain confidentiality and handle sensitive information.
• Organizational and documentation skills with follow-through.
• Ability to travel throughout Sussex County.

Responsibilities

• Conduct community-based outreach and engagement throughout Sussex County.
• Build trusting relationships using trauma-informed, low-barrier approaches.
• Provide harm reduction education and distribute approved supplies.
• Deliver overdose prevention education, including naloxone information.
• Perform SBIRT screenings per training and procedures.
• Provide care navigation to treatment, recovery, healthcare, and services.
• Offer warm handoffs to outpatient treatment and recovery services.
• Support rapid assessments and clinical connections as needed.
• Assist MOUD access and ongoing recovery supports when appropriate.
• Connect participants with housing, workforce development, primary care, and behavioral health services.
• Follow up on referrals and address barriers to engagement.
• Develop relationships with treatment providers and referral partners.
• Represent Impact Life at outreach events and community sites.
• Maintain documentation of outreach contacts, screenings, referrals, and engagement.
• Collect program data to support grant outcomes.
