Job summary

Molina Healthcare is seeking a Nurse Practitioner to provide screening, preventive primary care and medical services to members in non-clinical settings, including in-home, community sites and pop-up clinics. The NP will coordinate care, document findings in EMR, order tests and collaborate with physicians to develop comprehensive plans of care.

Travel within the assigned region up to 120 miles daily, participate in community outreach, and support efforts to improve access to high-quality care

Qualifications

• Active and unrestricted FNP license in state of practice.
• DEA license with authority to prescribe.
• AANP or ANCC certification as required.
• BLS certification required.
• EMR systems experience.
• Strong communication and time-management skills.

Responsibilities

• Provide general medical care and care coordination to member populations.
• Perform history and physicals to identify gaps in care and preventive needs.
• Order and interpret diagnostic tests and develop care plans.
• Coordinate post-discharge care to reduce readmissions.
• Travel within assigned region up to 120 miles daily as needed.
