Job summary

Paradigm is seeking a Field Nurse Case Manager to cover the San Diego area in CA. This home-based role involves travel up to 2 hours one-way (4 hours round trip) for appointments at doctors’ offices, hospitals, and other sites.

The nurse will manage work-related injury cases from assessment through evaluation, coordinating with insurers, providers, attorneys, employers, and employees to drive meaningful outcomes and report progress to stakeholders.

Qualifications

• Current, unencumbered RN license in California.
• Experience in case management of workers' comp cases preferred.
• Certifications CCM, CRRN, CDMS, CRC preferred; within two years of hire date.
• Strong organizational and communication skills required.

Responsibilities

• Manage medical case management of work-related injuries, including assessment, planning, implementation, and evaluation.
• Coordinate with insurance carriers, medical care providers, attorneys, employers, and employees; monitor progress and report results.
• Provide autonomous, patient-centered education and support during treatment and return-to-work processes.
