Job summary

Paradigm is seeking a Field Nurse Case Manager to cover San Francisco, CA and surrounding areas. This is a home-based role with travel to doctors’ offices, hospitals and other sites, requiring assessment, planning, coordination, implementation and evaluation of injured workers’ case management.

The successful candidate will hold a CA RN license, with preferred workers’ comp case management experience and relevant certifications; strong organization, communication, and patient education skills

Qualifications

• Active California RN license is required.
• Experience in workers' comp case management preferred.
• CCM/CRRN/CDMS/CRC certs strongly preferred and must be obtained within 2 years.
• Strong organizational and time-management skills.
• Good verbal and written communication; patient education focus.
• Valid driver's license with clean driving record.

Responsibilities

• Manage medical case load for work-related injuries from assessment to evaluation.
• Coordinate care with insurers, providers, attorneys, employers and employees.
• Monitor progress and report outcomes to employers and carriers.
• Develop and implement care plans and track timelines.
