Qualifications

• Active RN or CRC license required.
• CCM and/or CIRS certifications preferred.
• Must comply with state licensure requirements.
• Prior workers' compensation case management experience preferred.
• Valid driver's license and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including workplace visits.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers and internal team members.
