ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work by coordinating care among medical providers, employers, and insurers. The role emphasizes timely, transparent case management and collaboration with diverse stakeholders.

This position requires an active NJ RN license and may suit nurses or certified rehabilitation counselors. Prior workers' compensation experience is highly valued, and travel to appointments is required.

Qualifications

• Active NJ Compact RN license is required.
• CCM or CIRS certifications are preferred.
• Prior workers' compensation case management experience is strongly preferred.
• Valid driver's license and ability to travel to appointments.

Responsibilities

• Coordinate care among medical providers, employers, insurers, and injured workers.
• Attend appointments with injured workers at various sites.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide regular progress reports to clients and stakeholders.
• Ensure case management meets customer and compliance requirements.
• Build strong relationships with clients, providers, and internal teams.
