Job summary

ForzaCare in California is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will coordinate care among medical providers, employers, and carriers, attending appointments as needed.

The role is ideal for licensed nurses or CRCs; prior workers' comp experience is preferred. Travel to appointments is required and you will maintain clear progress reports.

Qualifications

• Active RN or CRC license with a degree is required.
• CCM, CIRS or other case management credentials are preferred.
• Must comply with state licensure requirements.
• Prior workers' compensation case management experience is preferred.
• Valid driver's license and reliable transportation required.

Responsibilities

• Coordinate care between medical providers, employers, and insurers.
• Attend appointments with injured workers at their workplace or clinic.
• Develop, document and monitor recovery goals and return-to-work plans.
• Provide consistent progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance standards.
• Build and maintain relationships with clients, providers, and team members.
