Job summary

ForzaCare in Delaware is seeking a Field Medical Case Manager to coordinate care between medical providers, employers, insurers, and injured workers, ensuring timely, transparent, and effective care management. You’ll accompany appointments and help develop individualized recovery goals and return-to-work plans.

The role favors licensed nurses or CRCs, with strong preference for workers' compensation experience.

Qualifications

• Active RN or CRC license with associated college degree is required.
• CCM/CIRS or other case management credentials preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience preferred.
• Maryland WCCM preferred.
• Valid driver’s license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurers, and injured workers.
• Attend appointments with injured workers at their place of employment when needed.
• Develop, document, and monitor individualized recovery goals and return‑to‑work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
