Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate care for injured workers, liaise with medical providers, employers, and carriers, and support timely recovery and return-to-work planning.

The role is ideal for licensed nurses or Certified Rehabilitation Counselors, with prior workers' compensation experience valued. Valid driver's license and ability to travel are required; state licensure must be maintained.

Qualifications

• Active RN or CRC license with associated college degree is required.
• Prior workers' compensation case management experience is strongly preferred.
• Must comply with state licensure and certification requirements.
• Valid driver's license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers at various locations.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and internal team members.
