ForzaCare is seeking a Field Medical Case Manager to guide injured workers through recovery and safe return-to-work. You will coordinate with medical providers, employers, and insurance carriers, ensuring timely, transparent case management and clear communication with all stakeholders.

This role is ideal for licensed nurses or certified rehabilitation counselors; prior workers’ compensation experience is strongly preferred and travel to appointments is required to support the care plan.

Qualifications

• Active RN or CRC license with associated college degree is required.
• CCM, CIRS or other case management credentials preferred.
• Must comply with state licensure and certification requirements.
• Prior workers’ compensation case management experience strongly preferred.

Responsabilités

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments at workplaces or other locations as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide regular progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
