Job summary

CommonSpirit Health at Home seeks a Market Development Representative-Care at Home to drive growth by identifying referrals and educating communities in the Washington area. This field-based role focuses on relationships with skilled nursing facilities, hospitals, physicians, and other health care providers.

You will maintain key relationships, coordinate with branches, and help ensure patient transitions to home care services, while meeting monthly admission targets.

Qualifications

• Minimum of one year of related sales experience preferred.
• High school diploma required; Bachelor's degree, clinical license, or experience as a Care Transition Nurse preferred.

Responsibilities

• Identifies and qualifies health care relationships within regional territory with a focus on skilled nursing facilities, assisted living communities, community based residential facilities, physicians and other health care providers and hospital systems.
• Educates the community about services.
• Develops and maintains relationships with key customer sources through regular contact and follow-up procedures.
• Acts as the key contact for client troubleshooting and conflict resolution.
• Maintains accurate data on customer relationships within RSL to provide client information needed to build strong ties and deliver customer satisfaction.
• Communicates to branches daily to share vital client and patient information.
• Meets or exceeds monthly admission targets as provided by supervisor.
• Assists with unbilled revenue and Face to Face as requested by branch leadership.
• Follows referral of a patient to the organization and assists discharge coordinators in coordinating care quality home care services.
