Job summary

Elevance Health is seeking a LTSS Service Coordinator – Case Manager to support a defined caseload across designated Florida counties. The role centers on assessments, person-centered planning, and coordination of LTSS/IDD and behavioral health needs.

The position is field-based with required travel, primarily in the EST zone; compensation includes a non-management, non-exempt track with a Monday–Friday schedule and hybrid expectations where applicable.

Qualifications

• Requires BA/BS degree and two years of experience with a social work agency or equivalent.
• Bilingual in English/Spanish preferred.
• LTSS, case management, social work or hospital discharge planning experience preferred.

Responsibilities

• Conduct face-to-face program assessments.
• Coordinate and manage member waivers (LTSS/IDD) and BH/PH needs.
• Document goals and develop a Person Centered Support Plan (PCSP).
• Engage care teams and relatives to ensure integrated care.
• Submit utilization/authorization requests and participate in audits.
