Job summary

Elevance Health is seeking a LTSS Service Coordinator (Case Manager) to manage a designated Ohio caseload and lead person-centered planning. The role travels to client sites and requires field-based engagement with occasional office attendance, assisting with LTSS/IDD needs while aligning with state regulations.

You will conduct assessments, document PCSPs, and collaborate with medical directors and an interdisciplinary team to integrate care.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience with a social work agency.
• Strong case management experience with older adults or individuals with disabilities.
• Experience coordinating care with interdisciplinary teams.
• Proficient with person-centered planning and documentation.

Responsibilities

• Perform face-to-face program assessments using predefined questions and motivational interviewing.
• Identify members with potential healthcare needs and coordinate their cases with the clinical team.
• Manage non-clinical needs of members with chronic illnesses to optimize utilization of LTSS/IDD services.
• Document short- and long-term goals in collaboration with the member’s care team.
• Support training of new staff and participate in process improvement initiatives.
