Job summary

Elevance Health is seeking an LTSS Service Coordinator to manage a defined caseload, conducting assessments and developing Person Centered Support Plans. The role involves field work, collaboration with care teams, and interfacing with medical directors to ensure integrated care plans.

Hybrid work arrangement applies, with travel to client sites as needed; candidates outside commuting distance may be considered with accommodations as required by law.

Qualifications

• BA/BS degree.
• Minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments for LTSS/IDD and BH or PH needs.
• Identify members with potential clinical health care needs and coordinate with the clinical team as the single point of contact.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective LTSS utilization.
• Document member goals in collaboration with the care team.
• Identify members who could benefit from alternative levels of service or other waiver programs.
• Mentor or precept new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies.
• Assist with appeals, grievances, and state audits.
