Job summary

Flournoy Health Systems is seeking a Registered Nurse (RN) to provide clinical oversight and coordination of home and community-based care within assigned service areas. You will assess, plan, document, and guide care teams to support safe, effective, and compliant patient care.

Qualifications

• RN with clinical oversight and care coordination experience.
• Strong documentation and HIPAA compliant practices.

Responsibilities

• Complete initial assessments and reassessments and maintain individualized Plans of Care for clients receiving home-based services.
• Provide clinical supervision and guidance to LPNs and caregivers, including competency validation.
• Coordinate with the operational team to schedule supervisory visits and ensure required visits are completed on time.
• Conduct supervisory visits for skilled clients and other visits as clinically appropriate.
• Review and sign off on clinical documentation to ensure alignment with the Plan of Care.
• Manage physician Plans of Treatment, including coordinating orders and documentation.
• Monitor changes in patient condition and coordinate appropriate clinical interventions.
• Communicate with physicians, case managers, families, LPNs, caregivers, and operational teams to support continuity of care.
• Educate clients and caregivers about diagnoses, disease processes, medications, treatments, safety, and care needs.
• Ensure services and documentation comply with Medicaid, HIPAA, and agency requirements.
• Provide direct nursing coverage as needed to address staffing shortages and maintain continuity of care.
