Rotech Healthcare is seeking a Hospital Liaison to coordinate discharges from hospital to home-based respiratory and DME services. You will collaborate with Case Management and Continuing Care teams to ensure smooth transitions and timely equipment delivery.

The role involves education for patients and caregivers, travel to patient homes, and participation in quality improvement initiatives. A valid driver’s license and healthcare coordination experience are preferred.

Qualifications

• Requires ability to coordinate patient discharges with hospital staff and home care teams.
• Strong communication and documentation skills in a healthcare setting.
• Experience with medical equipment or healthcare administration is preferred.

Responsabilités

• Coordinate discharge planning between hospital staff and home care services.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain accurate documentation including referrals and delivery tickets.
• Travel to patient homes for equipment setup and follow-up when required.
