Job summary

Rotech Healthcare Inc is seeking a Hospital Liaison to bridge hospital teams and home care services. You will coordinate discharge planning for patients needing respiratory therapy and DME at home, ensuring timely equipment delivery and patient education.

The role involves travel to patient homes, collaboration with Case Management, and maintaining accurate documentation. A valid driver’s license and healthcare coordination experience are required.

Qualifications

• Experience coordinating healthcare transitions and discharges.
• Ability to educate patients and caregivers on equipment use.
• Must have valid driver’s license and clean driving record.

Responsibilities

• Coordinate patient discharges with hospital case managers and care teams.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on proper equipment use and maintenance.
• Maintain documentation including referral forms, insurance verification, and delivery tickets.
• Travel to patient homes using a company vehicle.
