Job summary

Progressive Home Health Inc. is seeking an experienced Hospice RN Field to provide professional nursing care to home care patients as prescribed by physicians and agency policies.

You will lead interdisciplinary team discussions, manage care for up to 16 patients, and develop individualized plans updated every two weeks. The role requires an active Oklahoma RN license, 2+ years hospice experience, strong communication skills, and willingness to travel within the service area under varying

Qualifications

• Active Oklahoma RN license.
• 2+ years hospice nursing experience preferred.
• Recent direct patient care in hospice or institutional settings preferred.
• Strong verbal, written, and electronic communication skills.
• Willingness to travel within service area under varying weather conditions.

Responsibilities

• Lead and participate in interdisciplinary team meetings and case discussions.
• Manage care for up to 16 patients across different settings.
• Develop and implement individualized care plans updated at least every 15 days.
• Perform admissions, routine skilled visits, evaluations, and on-call visits.
• Supervise hospice aides with visits and training.
• Maintain compliant documentation and Medicare-standard processes.
• Coordinate care with facility staff, physicians, pharmacies, and other healthcare partners.
