Job summary

Haven Home Health + Hospice seeks a Hospice RN Field PRN to support home-based and institutional care settings in Norman, OK. The role delivers skilled nursing care under physician orders, adhering to state nurse practice acts and agency policies, with travel within the service area as needed.

Responsibilities include coordinating with the interdisciplinary team, managing up to 16 patients, and developing individualized care plans updated every two weeks or as patient needs evolve.

Qualifications

• Active RN license in Oklahoma; hospice experience preferred.
• Strong communication skills, both written and verbal.
• Willingness to travel within the service area and adapt to changing tasks.

Responsibilities

• Lead and participate in interdisciplinary team meetings and case discussions.
• Manage care for up to 16 patients across different settings with clear goals.
• Use critical thinking and strong communication in patient care.
• Develop and update individualized care plans at least every 15 days.
• Perform admissions, routine visits, evaluations, imminent visits, and PRN/on-call duties.
• Ensure compliance with policies and Medicare requirements.
• Supervise hospice aides with visits at least every 14 days and provide training.
• Conduct monthly supervision of LPNs, including home visits and chart reviews.
• Support routine, respite, continuous, inpatient, and imminent visits.
• Maintain accurate, timely, complete documentation per standards.
