Job summary

VitalCaring is seeking an experienced Hospice RN for field-based, patient- and family-centered care. You will lead plans of care, manage symptoms, and deliver comfort-focused services aligned with patient goals in home and facility settings.

Qualifications

• Active RN license (or compact eligibility).
• Minimum 1 year of clinical nursing experience.
• Graduate of an accredited nursing program.
• Strong clinical judgment and compassionate care.
• Ability to manage a caseload independently.
• Excellent communication in sensitive situations.
• Valid driver's license and reliable transportation.
• Basic EMR proficiency.

Responsibilities

• Lead Patient-Centered Hospice Care with comprehensive assessments and individualized plans.
• Serve as Care Coordinator, coordinating care across the interdisciplinary team.
• Support patients and families with education, empathy, and guidance.
• Drive quality and compliance through accurate documentation and care planning.
