Haven Home Health and Hospice is seeking a Hospice RN Field to provide professional nursing care to home care patients as prescribed by the physician and in accordance with state regulations and agency policies. The role requires Oklahoma RN licensure, hospice experience preferred, and willingness to travel within the service area.

You will collaborate with an interdisciplinary team, manage patient care plans, and ensure accurate documentation across various settings.

Formación

• Active RN license in Oklahoma.
• 2+ years hospice nursing experience preferred.
• Recent direct patient care experience in hospice or institutional setting preferred.
• Strong verbal, written, and electronic communication skills.

Responsabilidades

• Lead and participate in interdisciplinary team meetings and case discussions.
• Manage care for up to 16 patients across settings, ensuring needs and goals are met.
• Perform RN duties including admissions, routine visits, evaluations, and on-call visits.
• Supervise hospice aides with visits at least every 14 days and address concerns.
• Coordinate care with facility staff, physicians, pharmacies, and other health professionals.
• Maintain thorough, compliant documentation reflecting clinical judgment and Medicare standards.
• Attend interdisciplinary team meetings every 15 days and care coordination meetings.
• Provide counseling to patients and families and educate hospice staff.
• Uphold ethical standards and comply with hospice laws and workplace regulations.
