Job summary

Ohio Living is seeking a Registered Nurse to join our Hospice Field Staff, delivering in-home and facility-based care. You will assess, plan, and implement nursing services, ensuring compliance with standards and regulations, and collaborating with the care team to manage pain and symptoms for patients and support families.

Responsibilities include creating comprehensive care plans, documenting care, and educating patients and caregivers while adhering to agency protocols and quality guidelines.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
• One year experience in acute care or medical-surgical nursing required.
• Experience in a Medicare-certified home health agency preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
• Proficiency with electronic medical records (EMR) preferred.

Responsibilities

• Provides ongoing assessment and nursing care to patients in homes and health care facilities.
• Develops and implements a comprehensive care plan addressing current needs.
• Follows nursing standards of practice and hospice protocols.
• Manages pain and symptoms for patient comfort.
• Coordinates with health care providers about changes in condition and needs.
• Educates patients and caregivers as needed.
• Prepares for IDG by reviewing Care Plan and reconciling medications.
