Job summary

VitalCaring Group in Oklahoma is seeking a Hospice RN to lead patient-centered end-of-life care in homes and community settings. As a field-based clinician, you’ll provide comfort, dignity, and compassionate presence while coordinating with an interdisciplinary team.

You’ll lead plans of care, manage symptoms, educate families, and document care with accuracy. The role requires an active RN license and at least 1 year of clinical nursing experience, plus reliable transportation for field visits.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver's license and reliable transportation.

Responsibilities

• Lead the plan of care and manage symptoms.
• Coordinate care with physicians, social workers, chaplains, and aides.
• Provide education and emotional support to patients and families.
• Maintain accurate documentation and participate in care planning.
