Job summary

Progressive Home Health, Inc. is seeking a Hospice RN Field to provide professional nursing care to home care patients, ensuring compliance with the State Nurse Practice Act and agency policies. The role may also deliver nursing services in institutional settings.

The RN will lead interdisciplinary team meetings, manage care for up to 16 patients, develop individualized plans, and supervise hospice aides and LPNs. Travel within the service area is required.

Qualifications

• Active RN license in Oklahoma.
• Hospice nursing experience preferred.
• Direct patient care in hospice or institutional settings preferred.
• Strong verbal, written, and electronic communication skills.
• Ability to adapt to frequently changing tasks.
• Valid driver's license and willingness to travel within service area.

Responsibilities

• Lead and participate in interdisciplinary team meetings and case discussions.
• Manage care for up to 16 patients across settings, meeting patient needs and goals.
• Use critical thinking, decision making, and effective communication in patient care.
• Develop and update individualized care plans at least every 15 days as patient needs evolve.
• Perform RN duties including admissions, routine visits, evaluations, imminent visits, and on-call/PRN visits.
• Ensure compliance with policies and directives from leadership.
• Supervise hospice aides with visits at least every 14 days, addressing concerns through observation and training.
• Conduct monthly supervision of assigned LPNs, including home visits and chart reviews.
• Provide support for all hospice levels: routine, respite, continuous, inpatient, and imminent visits.
• Maintain thorough, compliant documentation with timely electronic entries and Medicare-aligned narratives.
